Gather the following items before calling Verizon:

  • Proof of identity. You're going to have to provide a government-issued photo ID.

  • Police report. Verison's policy is to cooperate fully with the entirety of their criminal investigation. You therefore must provide the police with as much information as possible in the form of a jpeg, png, or pdf.

Things You Will Need for Business Cases

For businesses, you will need two forms of ID. Firstly, a form of government-issued photo ID from the appropriate party. Secondly, a form of identification for your business from the list below:

  • A valid business license
  • Registry of a non-profit organization that has been issued by the attorney general
  • Notarized business lease, proof of address is the official documentation for the business's main address, this helps Verizon to validate your business against the account
  • The first three pages of Articles of Incorporation
  • Certificate of good standing/formation as issued by the Secretary of State. You must validate it first or provide a letter proving an update was requested.
