Click File a Claim

Is your device broken or cracked? Go to the file a claim page and click File or Track a Claim.

Do note that only the primary holder can file a claim with Assurant. Also, be sure to have all the required information ready before filing the claim. You need the make and model of your device, the last time you used it, and the date it was damaged.

Enter Your IMEI Number or Mobile Number

Type in your T-Mobile mobile number or your device's IMEI number. Then click Submit.

The IMEI is usually a 15-digit identification number or a static identifier. You can get this via the device settings or by dialing a code. Just go to About Phone if you have an Android device or About if you have an iPhone.

Just follow the on-screen instructions on how to file a claim with Assurant. You'll be able to also track the status of your claim online.

If your claim is approved, the replacement device is usually shipped overnight. Expect to receive it within two business days. However, do note that some claims may take several days.
