Despite several attempts to contact their customer service, I couldn't speak with anyone due to limited office staff. My current name is Edwin Mendez, having dropped my birth name Jalugue after a divorce in ****. I possess various official documents like my driver's license, SSI card, and more, which confirm my identity change. I can provide these documents upon request. VitalChek declined my online order citing my name change, although I used my US Bank debit card for the purchase. My driver's license expires soon, so I need this matter resolved promptly.

To whom it may concern, I have been trying to resolve an issue with my birth certificate order placed in January [redacted], but keep receiving unhelpful automated responses from both DOH in NY and Vitalchek. I recently placed an order for one certified copy of my birth certificate through Vital Check online. Although I only received one copy, I noticed that I was charged for two copies, including shipping, handling, and processing fees. On April 28, my Visa was charged $64.95 for the initial birth certificate, which was fine. However, on May 18, my account was charged an additional $69.95 for a second birth certificate that I did not order. Vital Check deducted both amounts from my provided card. I am requesting a refund of $69.95 for the unordered birth certificate, and I hope to see this amount returned to my bank account promptly. I have not received a response from Vital Check yet. My wife has been waiting for her birth certificate since February 28th and has been unable to get her license renewed or obtain an ID due to the delay. This situation is preventing her from driving and getting a job, causing a lot of frustration. Despite already paying for the birth certificate, she has not received any updates on the status. I recently placed an order for a CA birth certificate with the order number #[redacted]33PIN [redacted]. Unfortunately, during the finalization process, I mistakenly selected an option that now requires me to visit a notary public. Is it possible to rectify this so I can simply provide a copy of my state-issued ID? I am hoping to expedite this process by submitting a photo of my state ID to move my order forward. Additionally, I find the pricing of your services to be quite high, especially as I am on a limited income due to a disability. It would be beneficial if your company could offer discounts to individuals facing financial challenges. The cost of overnight shipping also struck me as excessive; perhaps including second-day service would be more reasonable given the overall price.

I would greatly appreciate an update on the status of the birth certificate for William Grant Staples. If possible, please provide me with a tracking number via email. Hello, my name is Olga Rolon. I purchased a copy of my mother Eleuteria Menendez's birth certificate from Vega Baja, PR, on your website with certificate number A[redacted]. I bought this certificate to correct the spelling of my mother's name on my birth certificate in November [redacted] based on the advice of the Bureau of Vital Records. After purchasing it, I uploaded a scanned copy of my mother's ID as requested, but was later informed in January [redacted] that it was not received. Following a phone call, I faxed the ID to the provided fax number, which was confirmed received by your representative. However, I have yet to receive the birth certificate in the mail. Due to the pandemic, I am unable to contact anyone for assistance. I have provided all information requested and paid for the service. I am unsure of my order number to check the status on your website. How can I obtain my mother's birth certificate by mail at this time? Should I submit another request and pay again? I purchased 5 death certificates on vitalchek.com, paying $83.30, without receiving them after 38 days since the order and 24 days since they claimed the process was complete. I am requesting an immediate delivery of the certificates I paid for or a full refund promptly. VitalChek has advised that the order information was sent to New York City Dept. of Health and Mental Hygiene, currently processing the request. An email will be sent once the certificate ships. On June 15th, I placed an order with NYC VR in Brooklyn for two birth certificates to be shipped to Mexico, where I currently reside. I have included the transaction details below for reference. Unfortunately, regular mail delivery in Mexico is unreliable, and there is a risk of mail being stolen. I am unsure if the delay of 6 weeks is normal or if the certificates may have been lost in transit. In case the birth certificates are indeed lost, I would like to inquire if they could be re-sent using FedEx or DHL with the shipping charges billed to my credit card. I purchased a birth certificate for my mother from Los Angeles. When she tried to use it at the DMV to get her driver's license, we discovered that it was labeled "for information purposes only," not the original copy we needed. We are seeking assistance to obtain the correct original birth certificate as we shouldn't have to pay for it again since the first one was incorrect. I made a mistake with the delivery address for my certified birth certificates. I mistakenly entered [redacted] NE 17th Court Unit 8 instead of [redacted] NE 17th Court Unit 8, Fort Lauderdale, FL [redacted]. UPS wasn't able to deliver the package, and I couldn't pick it up due to the address discrepancy with my driver's license. VitalCheck advised me to change the address with UPS, but your company's requirements prevented me from doing so. I've attempted multiple times to correct the address via email with VitalCheck. The UPS tracking number is 1Z1F[redacted][redacted], and my VitalCheck order number is [redacted]32. The package has been returned. I would like the address corrected and the package reshipped to the accurate address. I'm willing to cover the reshipment charges with the original credit card. On July 13, [redacted], I contacted the Department of Vital Records in Raleigh, NC to request a copy of my divorce certificate. The representative I spoke to obtained all my details, including my credit card information, and informed me of additional charges for expedited processing and overnight delivery. Despite paying $86.70 for these services, it has been exactly one month, which is 23 business days, and I have yet to receive the document. Interestingly, I had also ordered a birth certificate from VitalChek online subsequently, opting for regular processing, and already have it in hand. I am seeking a refund for the expedited and overnight services I paid for. I am willing to provide a screenshot of the credit card transaction upon request.

I am concerned about my personal information. I placed an order a month ago and was advised it would be processed within 6-8 days. Unfortunately, I have not received any updates and have been unable to reach anyone for assistance. I am uncertain if my order was delivered, lost, or if there is an issue. Despite multiple calls with long wait times, I have yet to speak with anyone. Today, I have been on hold for nearly an hour again. I also emailed my concerns but have not received any response. On Wednesday, August 19th, I requested a copy of my marriage certificate from April 15, [redacted], for F. G. Brown and R. D. White. I paid for expedited shipping to receive it by Friday, August 21st. I received an email indicating the estimated delivery date, but as of today, August 28th, I have not received the certificate or any further updates. When trying to check the status on the website, I encountered difficulties. The message I received stated they could not retrieve the status of my order and advised me to call customer service. However, when I called 1-[redacted], I was directed back to the website, creating a frustrating loop with no resolution.

The representative requested my order number and then asked for my PIN number, which I hesitated to provide as per the email instructions. However, he assured me they needed it to process my application, so I reluctantly shared it. I encountered difficulties submitting my information and suspect I may have been misled into sharing sensitive details. Additionally, the representative mentioned that my application was being handled by the State instead of VitalChek. I consistently spoke with the same man, possibly named Don, whose actions have left me feeling uneasy and vulnerable. I urge vigilance to prevent any unauthorized access to my birth certificate.
