Nearly a year ago, I applied to renew my passport and my twin grandkids' passports, both aged 14, at the local post office. While I have received my renewed passport, I am still waiting on my grandkids' passports due to complications. Despite speaking with a representative and following their instructions to reply to a denial letter and provide a notarized biological parent's approval letter, I recently received another denial letter for the same issue. This delay has impacted our plans for a Caribbean cruise to celebrate their birthday in April [redacted]. I have tried calling multiple numbers to resolve this matter but have been unsuccessful in speaking with a live person. Any assistance with this ongoing passport issue would be greatly appreciated. Thank you, Jennifer Polvere.

I made a mistake on my passport application by printing my name instead of signing in cursive. Now, I've received an email stating that I need to send in a "completed" application. The issue is that I already sent my old passport with the original application, and it has my passport number on it. Without that information, I can't fill out a new application completely. I'm unsure how to rectify this to ensure I receive my passport in time for my upcoming trip abroad.

I need assistance regarding the validity of my passport.
Last year, I believed my passport was stolen during a break-in at my home. However, I recently located it, but I am unsure if I reported it as stolen at the time. I hope to avoid replacing it if it is unnecessary. The passport number is [redacted]96, and the passport card is C[redacted]7, both valid until 19 Sept [redacted].
After reaching out via mail and email to the passport agency and receiving no response, I am concerned as I have an upcoming international trip in five weeks.
I require clarification on whether my current passport is suitable for international travel or instructions on obtaining a replacement before my journey.

I need assistance determining if my passport is still valid for international travel. Last year, I believed it was stolen during a break-in, but I recently located it and am unsure if I reported it as stolen. The passport number is ********* with an additional passport card numbered C********. Both documents expire on ** Sept ****.Unfortunately, I have not received any response after contacting the passport agency through written correspondence and email. With an upcoming international trip in five weeks, I urgently need to confirm the validity of my current passport or understand the process for obtaining a replacement before my travels.

I recently applied for a passport as a new citizen who is also a divorcee, and my name was changed back to my maiden name by the court. I included the supporting divorce decree, which states this name change on the second page, as requested in the application instructions for proof of name change. Additionally, my citizenship was originally issued in my maiden name, and I submitted the original Citizenship Certificate. The passport office is requesting another document in my maiden name, but I am unsure if they saw the court document showing the name change back to my maiden name. I followed all the guidelines on the application form by submitting the court document and my Citizenship Certificate. I am hoping the passport office can acknowledge the court document and my citizenship certificate so I can proceed with my application smoothly.

I am about to renew my passport, which expires next year. I have filled out the form, got a passport photo, and prepared the fee to mail to the address provided. I have a question regarding the form, though.
During my last passport application ten years ago, there was a mistake. Although my first and last names are correct, my middle name is wrong on the passport. As I renew my passport, I am unsure if I can correct the middle name to match the accurate one.
The form has a section to enter my name as it appears on my current passport. Should I input my correct middle name despite it being different from what is on the current passport?
Even if I submit a marriage certificate, it won't reflect my legally changed name since it's not mentioned on the certificate.
To clarify, my legal name is Rachel Garrett Bagley, while my current passport shows Rachel Jane Bagley. How can I update my passport to feature the correct middle name?
Thank you for your assistance!

Hello, my name is Vala Peitz. My passport locator number is [redacted]51. On April 25th, I submitted a request to the State Department for an expedited passport. On April 29th, I was informed that the IRS had denied my passport due to a tax issue. After resolving the tax matter on May 11th, the denial was reversed by the IRS on May 23rd, and the State Department was notified. I have received confirmation of this reversal. However, as of June 17, [redacted], the State Department is still processing my passport. I have a flight booked to Greece on July 8th with my disabled son to meet his brother on a religious retreat. I'm concerned because the passport delay may leave my son's brother stranded without a phone in a foreign country. Despite numerous attempts, we have not been able to expedite this process. Your assistance is urgently needed.

Hello,
I am Kylar Bryan Harvey and I have applied for a US Passport twice already. I was born in Brooklyn, New York, in [redacted]. Initially named Baby Girl Bryan by the state of New York as my mother had not named me at birth, I was later named Kylar Kwan Bryan. I have had a social security number and California ID all my life under my given name. Despite initially being denied a passport for only having a copy of my birth certificate, I obtained my original birth document since and submitted a notarized Birth affidavit with all required documents. I am now being asked by the state for the same birth affidavit again, along with additional documentation. I am puzzled about what more documents are needed. I have provided all necessary information and have a stable residency and work history. As a US citizen trying to reunite with family in Africa, I am seeking guidance on how to proceed with my passport application. Thank you for your assistance.
Regards,
Kylar B. Harvey

I recently applied for my children's passports, and I received an email from the National Passport Center indicating that there was an issue with the social security numbers provided in their applications. Our children, who were adopted recently, have their current legal names on the applications, but the social security numbers seem to be associated with different last names. Richard Michael's (RE: [redacted]81) last name is Palladino on the application, but the social security number is linked to the last name Vigorito. Similarly, Olivia Virginia's (RE: [redacted]80) last name is Palladino on the application, but the social security number is linked to Palladino. Lastly, Michael Vincent's (RE: [redacted]79) last name on the application is Vigorito, but the social security number is associated with Palladino. Rest assured that all other details in the applications are correct. Your prompt attention to this matter is greatly appreciated as we need the passports for our children.

I submitted a passport application 2 months ago. Just yesterday, I received a letter requesting documents I do not have. I was born to American Citizen parents in American Samoa, and I do not have a Certificate of Citizenship, my parents' birth certificates, or a notarized letter detailing their residence history. Moreover, both of my parents have passed away, making it impossible to obtain this information. I am unsure of how to proceed given these circumstances.

Hello, my name is Norma Ramirez. A few months back, I applied for a passport and had concerns about my birth certificate showing my middle name before my first name. Throughout my life, I've always been known as Norma. Unfortunately, when I requested my birth certificate from my ex-husband's house, where I left it, it was not there, so I had to obtain a new one. However, when I received the new certificate, it displayed my name as Louisa Norma instead of Norma L. Hernandez, which is how I've always been known. My marital name is Ramirez, and even though I didn't change it after my divorce, the postal worker assured me it would be acceptable for my passport application, which turned out to be false. I would appreciate the opportunity to speak with someone to clarify my unique situation. Thank you.

I recently applied for a passport as a US citizen in my 70s on 8/23/22. When I received a letter from the Passport Center in Portsmouth, NH, instead of in California, I realized there was an issue with my birth certificate. After obtaining the correct birth certificate and sending it to Portsmouth on 10/14/22, I managed to schedule an appointment in Los Angeles on 10/18/22. While everything went smoothly during the appointment, my birth certificate still hasn't been updated online. Unfortunately, I missed joining my family on vacation due to these delays. Despite the kind but unactionable responses from the help line, I am concerned about the status of my application with Portsmouth. An email dated 10/20/22 informed me that my file is being processed with a 4-week wait time. I've realized the importance of following all guidelines and preparing all necessary documents and photos before applying for a passport to avoid delays. If possible, I recommend visiting a Passport Center in person for a smoother process.

Dear Forum Members,
I am seeking assistance for my grandson's passport application. His father does not possess a photo ID, which is required for the process. Despite having an expired yearbook photo, a Social Security card, and an old college ID, they have been deemed insufficient for the application. As such, we are unsure of our next steps in obtaining the passport before the end of December. Any guidance or advice would be greatly appreciated.
Thank you,
Heather G.

I recently applied for my grandson's passport. However, the Department of State sent me a letter stating that the application was only signed by one parent/guardian, even though we believe both of us signed it. They requested additional documents, including a court order specifically permitting us to apply for him, a court order granting sole legal custody to the applying parent (which we provided), or a court order authorizing the minor to travel outside of the United States with the applying parent. We already sent the court order showing we have all rights and responsibilities of a legal parent. I'm unsure of what more can be done. It's puzzling why they didn't return the application for the other person to sign if that was the issue. I tried contacting customer service but couldn't get through to a human agent.

I recently applied for a passport. Upon checking, there seemed to be outstanding child support payments linked to my application. To rectify this, I promptly paid off the balance in August [redacted]. The child support agency confirmed the payment to the passport office at the beginning of September. Surprisingly, I received a rejection letter on October 14th, [redacted], stating that my child support issues were unresolved, which isn't the case. Feeling unjustly denied, I am eager to speak with someone at the passport agency to resolve this matter and obtain my passport. Any assistance in resolving this issue would be greatly appreciated. Thank you. - T. H.

I mistakenly signed my DS-11 form with my alias instead of my legal name. I have prepared a Certified notarized letter to clarify that I have not legally changed my name to the alias and that I do not intend to use it as my signature.
I would appreciate guidance on the next steps. Can I schedule an appointment at the Headquarters to submit my letter, or is there a specific form I should use for this purpose?
I had a passport back in [redacted], but I have since reapplied.
Thank you,

I recently applied for a passport renewal with reference number [redacted]19. I requested both the Passport Book and the Passport Card. Unfortunately, I can only find my Passport Book as during a recent move after 40 years, my Passport Card got misplaced among some papers and I am unable to locate it. I received a letter from your agency requesting the old Passport Card, which I am currently unable to provide. I am seeking guidance on how to proceed in this situation.
Thank you for your assistance.

I am having trouble reaching the passport agency's toll-free number as it only directs me to an automated system, preventing me from speaking to a representative. The new passport we received for my son does not include the hyphen in his last name, despite all the supporting documents displaying the hyphen (such as his previous passport and driver's license). I am uncertain if the passport agency has eliminated hyphens from their records, similar to some airlines. I am unable to clarify this due to the phone system. If this is an oversight, I may have to resubmit the paperwork and incur the expedited fee again, which does not seem fair since the mistake was not mine. I am seeking assistance on how to communicate with a staff member who can address these concerns before proceeding with a new application.
