I have Guardian Dental DMO through my retiree plan with the Cook County (Illinois) Pension Fund. I am currently with Family Dental Insurance in Evergreen Park, but I am looking to switch providers due to their service issues. I requested a list of dentists in my area from Guardian over two months ago, but I have not received it yet. I do not want to rely on finding a dentist online as my laptop has issues. I just need the list sent to me, please. Two years ago, Family Dental repeatedly contacted me for an appointment, but on the day of the appointment, they canceled last minute claiming my dentist was not available when it was actually with the hygienist. They offered me lunch as an apology but have not reached out since. I expressed my dissatisfaction with their service, but there has been no further communication from them.

I am requesting a refund for the premiums we have paid due to an error in admissions and the non-disclosure of the missing tooth policy. If we had known about this policy, we would not have chosen this plan. We did not receive our ID cards and were informed that they were sent to the employer, who then passed them on to the only dentist we can see. This dentist is requesting a payment of $2,[redacted] for treatment that is not covered due to the missing tooth policy. It seems like fraudulent behavior to me. I will contact the insurance commissioner to understand the laws around this issue and submit a complaint. I have noticed online that others are also experiencing difficulties accessing treatment because they have not received their ID cards. Due to these challenges, my husband has decided to cancel our dental insurance as we cannot afford both insurance and out-of-pocket expenses. Had we known we would not be covered, we would have opted out of paying for his coworker's insurance. It is crucial that we receive our ID cards to seek proper treatment. I kindly request a refund of our premiums to avoid escalating this matter further. Thank you.

Regarding my dental care, last year I was under Guardian Dental with Member ID [redacted]46 and plan number G-[redacted]1. I am now retired and have switched to United Medical. My previous dentist, Dr. Gross, is listed under the Healthy Smile network but does not accept United Medical AARP. I have reached out to him by phone and mail without success. I have 5 implants, and my new dentist, Dr. Russell, requires the brand information of the implants used. Is there any chance an implant brand name is mentioned on any of the dental claims submitted? I will be joining Guardian Dental in the new year and am seeking advice on choosing the right plan. There are three options available: [redacted] - Dental DHMO, [redacted] - Dental PPO [redacted] Base, and [redacted] - Dental PPO [redacted] Buy Up, all offered through the Stagwell Group. My family consists of 7 members, including my wife, myself, and our 5 daughters who are all 15 and under. We are open to changing dentists. The monthly costs for these plans vary from $20 to $71, and I am unsure which plan would be the most economical for us, considering we are comfortable with switching providers and staying in-network. Any advice is appreciated.
